Adobe Acrobat Instructions
These forms are created using Adobe Acrobat. To use the forms you will need the Free
Acrobat® Reader which may be downloaded from their
website [simply click on the blue hyperlink above and follow the instructions.
This download may take several minutes depending on the speed of your
system.]. Please
make sure that you have the most recent update of Acrobat
Reader on your computer before attempting to fill out these forms.
This will ensure proper viewing and editing capabilities on
your screen. The quality of the printed form will be the same as the
original.
Instructions:

From the table of forms, choose the
one you wish to use. On the right hand column of the table, click
on that particular form's 'pdf' link. The file size is also displayed
for your information. Acrobat Reader will open automatically to display
it, and you should see the form on your screen.

Once the form is on your screen
you have several options. You may print the blank form or you may
fill in the information required on screen and then print the edited
version.
To edit the form on screen, simply click on the form
at the point you wish to enter information. For example, click on the
space for "Name" and a cursor will appear, simply type the name as it
should appear on the printed form.
Please NOTE: The check boxes on the forms
are configured to automatically be checked when you click on the box,
there is no need to enter an 'X' or other mark in these boxes. To move
to another space on the form you may either click the mouse on the desired
space or you may tab through the fields on the form. Should you make
a mistake in typing, you may return to any space to make corrections
by simply clicking the mouse on the desired space.
When you are satisfied with the edited form simply print
the form [choose the 'Print' option from the 'File' menu or click on
your 'Print' icon].
IMPORTANT: You will not be able to save the
edited version of the form in Adobe Acrobat Reader. Please print the
form and review it carefully for errors before going on to the next
form or exiting from the program.

the free Acrobat Reader), you will be able to save your edited form
on your computer.]
